This form is for one child. If you wish to enrol more than one child in the Confirmation program please complete a form for each child.

For those whose child completed First Reconciliation and First Holy Communion at a different parish,
please ensure you have a copy of the following before completing this form :
1. your child's Baptism certificate
2. Certificate of Preparation for Reconciliation
3. First Holy Communion certificate
The documents must be in PDF format and will need to be uploaded before you can submit the form.

I agree to help my child prepare for their Sacraments by participating fully with them in the preparation program. This includes attending the Parent Information Night and the Commitment Mass, ensuring my child completes the lessons properly, and bringing my child to Mass to return their work.
